BE IT KNOWN, that for good and valuable consideration, and in payment of the sum of $_________(the “Purchase Price”), the receipt and sufficiency of which is hereby acknowledged, the undersigned____________________(the “Seller”) hereby grants, sells, and transfers to ________________________(the “Buyer”) the following dog (The “Dog”):

1.      Humane Care of the Dog.

The Buyer promises to provide the Dog at all times with adequate food, water, shelter, affection, and medical care. If the Buyer cannot care for the Dog, he/she may return Dog to the Seller at any time, no questions asked.

2.      Medical Conditions and Medications.

3.      Short Term Good Health Warranty.

[Except as otherwise provided herein,] The Dog is guaranteed to be in good health at the time of delivery. If, within 72 hours (3 business days) of the date of this sale, the Buyer discovers that the Dog has a life-threatening condition, the Buyer agrees to return Dog to the Seller with a letter from an examining licensed veterinarian. The Seller will refund the Purchase Price, return the Dog to the buyer in well condition, or exchange the Dog for a dog of the same breed, sex, approximate age, and value. If replacement dog is provided, the terms and conditions of this bill of sale will apply to that dog, and all references to the Dog herein shall be taken to mean the replacement dog. The Seller will not be responsible for any veterinarian bills. If the Buyer does not take the Dog to an examining licensed veterinarian within 72 hours (3 business days) of the date of this sale, this short-term good health warranty is null and void. 

*This contract does not cover hypoglycemia. Hypoglycemia is a disorder caused by low blood sugar. This disorder occurs mainly in toy breed puppies between 6 to 16 weeks of age. Hypoglycemia occurs when a puppy misses a meal and has the stress of a new home. This condition also occurs when the puppy is allowed to exert too many calories (puppy is allowed to play too much). A Puppy should never use more calories that it can replenish with food.

TO GUARD AGAINST HYPOGLYCEMIA:
Do not let puppy exert too much energy for the first few weeks.
Make sure puppy eats 3 times per day, and leave dry food out at all times.
Make sure puppy drinks enough water.
Keep puppy warm.

4. Spay/Neuter Contract.

    Buyer agrees to have Dog spayed or neutered before Dog is 6 months old, on this date______.      If Buyer chooses to NOT have Dog spayed/neutered, Buyer must pay the remaining balance on the Dog’s breeder price. Buyer is responsible for all legal fees involved in confiscation of the Dog.
